Small Business and DVBE Certification Instructions and Applications

The certification application process requires you to complete and submit a 3-page application and support documents as specified on the certification application.  Your certification application cannot be processed without these items.

Certification Application Instructions

1. Complete the 3-page application.  You may complete the application online, then print and sign it; or print the application and then fill it out manually.
   
   
   • If you choose the online method of completing your application, you must complete the ENTIRE application online before printing it. The application has been programmed to accept certain information based on your specific field selections. Changes made after printing the application could result in incomplete or incorrect information, which may delay your certification results.  Application data entered online (via Acrobat Reader) cannot be saved and edited later. You must print your completed application before closing Acrobat Reader.
     
   • If you are applying for both Small Business and DVBE certifications, complete the entire Small Business application and ONLY Sections 1A, 3, 4, and 5 of the DVBE application. You must submit the Required Support Documents specified on both applications. 
     
     
2. Required for all small business applicants:  In addition to the 3-page small business certification application, you must complete, sign, and include with your certification application the "Affiliate Information" form.  Disregard sections 5 and 6 of the certification application as the "Affiliate Information" form replaces these two sections.
   
   
3. Submit your completed and signed application, required support documents, and for small business applicants the completed and signed Affilate Information form to:

Small Business Certification Application , STD. 813 (Rev. 1/2002) and/or
Disabled Veteran Business Enterprise (DVBE) Certification Application, STD. 812 (REV. 1/2001).
