The Contract/Delegation Purchase Order (Std. 65) has been revised, effective July 1, 2003. This form is now titled Purchasing Authority Purchase Order (Std. 65) and is available on the Department of General Services' webpage at: /busmgmt/pco071603.htm
Management Memo 03-10 requires state agencies to report all CMAS orders over $250,000 to DGS within 5 working days of award using the "Notice of Contract Award" form. The "Notice of Contract Award" form is available at the following two links: WORD
