Local Agencies Addendum
 
 
 
Please complete the Local Agency Addendum as follows:
Page 1.  Insert the date and the agency's legal name.
 
Page 2. Section 7.  Provide the contact information.
 
Page 2.  Obtain the signature of an individual authorized to sign documents on behalf of the agency.
Each Local Agency Addendum must show the signer's authority to sign.  This can be accomplished as follows:
  • If the agency has an attorney, the attorney can sign the Local Agency Addendum in the space provided.  If this document is signed by the attorney, nothing further is required.  Please submit the document to U.S. Bank.
  • If the agency does not have an attorney, either a Board Resolution or a Certificate of Authority must be provided.
If providing a Board Resolution, the resolution must appoint the individual/s authorized to sign documents pertaining to the Cal Card Program.
 
If providing a Certificate of Authority, please complete as follows:
Section 1.  Enter the agency's legal name (as it appears on the Addendum) and Federal Tax Identification Number.
 
Section 2.  List the names and titles and obtain the signatures of individuals authorized to sign documents on behalf of the agency.
 
Section 3.  Indicate how many individuals are required to sign legal documents on behalf of the agency.
 
Section 5.  Enter all required information and obtain the signature of an authorized individual (often the Secretary or Assistant Secretary of the agency) to attest to the authority of the signing party.  Then obtain the signature of a second authorized individual to attest to the authority of the first.  Note: Two individuals are required to sign this section.  Parties signing the Addendum are not eligible to sign the Certificate of Authority.
 
Submit the Certificate of Authority with the Local Agency Addendum to U.S. Bank.
