DGS Emergency Debris Removal Services Master Service Agreement

The State of California, Department of General Services (DGS), is developing a Master Service Agreement (MSA) for Emergency Debris Removal Services to assist in the Southern California clean-up efforts resulting from the recent fires.

If you wish to be listed as a contractor on the MSA, please return the following:

  • Completed Exhibit A – Scope of Work requires contractor response on items 4 through 7
  • Signed copy of the Contractor Certification Clauses dated March 2007 (CCC 307)
  • Signed copy of the Standard Agreement (Std. 213)
  • One copy of your Certificate of Status from the Secretary of State, which can be downloaded at http://kepler.ss.ca.gov/list.html, if applicable
  • One signed copy the Payee Data Record (Std. 204) form

In an effort to provide a listing of available contractors in an expeditious manner, we ask that [signed] copies be faxed or scanned to the following e-mail address/fax number today if possible.  The listing of participating contractors will be available to end users starting tomorrow (October 31) and will be updated continuously.  A hard copy of any contract package transmitted electronically should be sent to the following address:
